Complete and Submit our Online Data Form or Download the form and fax it to 480-247-4860.
STEP2
Submit the Paperwork
Once you submit the Data Form, an FPN representative will complete all the paperwork for you – all you’ll need to do is review it, sign it, and send it back to them along with the last page of the Program Guide and a voided check of the account you want your funds deposited in.
STEP3
Welcome Email & Instructions
After receiving credit approval, you will receive a welcome email from FPN that will include:
• Your merchant numbers
• A user guide for using FPN
• Login instructions for accessing the gateway
• The necessary customer service numbers
